Forms

These are the forms that all participants will need to bring to AWESOME:

• Emergency information form (PDF, 52kB) - two copies of this must be handed in when booking in at reception. Unit leaders must also keep their own copies of health/emergency contact information for their units.
• Shooting form (PDF, 80kB) - this must be filled in for any young person wishing to participate in shooting activities.
